Hawaii Administrative Rules § 17-892.1-22, administered by the Department of Human Services, require at least one adult caregiver with a current pediatric first aid certificate and at least one with a current child CPR certificate — from a recognized certifying program — to be present at the facility whenever children are there. The rule accepts equivalent programs and does not prohibit online or blended delivery, so a course that pairs online learning with an in-person skills check qualifies. Certification is kept current on the program's cycle, commonly every two years.

Hawaii's child care rules are quiet on epinephrine and anaphylaxis: they provide only that physician-prescribed medication in its original labeled container may be given with signed parental authorization and kept out of children's reach, then returned to the family when no longer needed. As best practice, any child with a known severe allergy should have an epinephrine auto-injector on site and a written care plan so every caregiver knows the signs and the steps.
